Survey - Extra Fields File Upload
Using this module users can attach files along with their responses. By selecting the option, users can effortlessly choose and upload the desired file from their local devices. Additionally, the system supports the upload of multiple files if needed, by activating the 'Upload Multiple Files' option.

- You can easily upload a document file in the survey.

- Also, download the uploaded files from the review page.
In the survey view, to add a question, click on the 'Add a Question'.
Select the question type: Upload file and Click on the upload file option to upload a file. If you want to upload multiple files, tick the 'Upload Multiple File' option.
You can also upload document files from the website view.
Start the survey and fill up the details. To upload a photo click on
the choose file button and select the file and submit the survey.
Review page looks like below.
You have the option to download the uploaded document by clicking on the download icon.
